Smoking ages the skin, hair and nails of people who are long time smokers. At 30, you may look 40... at 60, you may look 80. It does this by restricting blood flow to places in your skin that need a healthy blood flow. Your skin starts to harden, and wrinkle in unnatural ways that only cosmetic surgery, collagen and botox can fix. It ages the organs in the same way, your lungs, liver, stomach, heart... they aren't getting the blood flow and oxygen they need to replace themselves either. And only surgery, transplants and bypasses are going to fix that as well.
I smoked, from the time I was 14 until I was about 23 and decided that I wanted to have a child. Prior to getting pregnant, I quit. I knew the risks and decided that it wasn't worth it for my child to have low birth rates, higher incidents of asthma and other allergies and under-developed organs. Funny thing was that I didn't consider the risk to my own health, I just thought of the risk of someone else. Part of the problem too, is that we lack the ability to take care of ourselves. We don't weigh the risks and unhealthy factors when making choices in our lives. That's why we have so many smokers, drinkers, drug-users, obese people, and other such preventable problems. All of these things can kill us, but we don't seem to factor those things in. I'm not sure why we don't, whether it's a feeling of immortality or just a lack of self-esteem but there has to be something that drives us towards bad habits. It may even be different for different people. I know a guy who can't help but eat, regardless of his high blood pressure, diabetes, bad back and other weight related problems. He's not sure what's caused it, but if you put food near him he can't help but eat it.
It's sad that we can't treat these peoples illness. Addiction is an illness, as is not being able to take care of yourself. Anything in excess, is bad. Even over exercising or over-dieting can be an addiction and can kill you. The ready supply of goods and services makes this problem worse. Fast food, an over abundance of cars, a liquor store on every corner... Gone are the days where tobacco was produced in small quantities and was harder to get, where people walked to work and school or took the bus for long distances. We don't cook our food from scratch anymore either, we get take out or buy pre-packaged foods with their high levels of chemicals, and easy methods of cooking. Back in the good old days, Grandma used to make a loaf of bread that was just devine... but she spent more calories making the bread than she did eating the piece or two from the loaf. People are lazy, and consumerism is the reason for this laziness. It's become too easy to consume goods at a high rate, and too hard to work for it. We have machines, and chemicals to replace hard work and healthy foods. We go for convenience rather than making the right choices.
All the logic has been taken out of our lives. We no longer know why we do the things we do, and can't explain any of it with any kind of rationality. What we do know is that it's harder to fix the problem than it is to ignore it. So we keep going along our merry way, no matter how stupid it looks.

You are a smart guy, you know you should quit for yourself. You need a little self-worth, and self-recognition. I can't help you quit smoking, but you can. Just think about what you are doing to yourself, and start seeing the things in yourself that other people admire about you. Fuck the past and whatever you've done... you can deal with that when you are healthy in your mind, heart and body. Right now, just focus on all of your good points and go from there. As for the running two-miles thing... you know you can do it, you did it in the military. You can do it again, you just have to start with going down the block and back, then a few feet further each day until you can make it around the block. Keep going and eventually you'll be running two miles a day and wondering why you didn't do it sooner. You have so many reasons to do it, and you want to do it... you just have to know you are worth it for you and no one else.
