So maybe not, but their situation isn't necessarily permanent, it's more of a healing stage.
IMO their game comes out of individual needs that once fulfilled, can help each one to move on and their ability to move on depends on whether their game starts to interfere with other strong needs - for instance if years from now the guy starts to want a normal relationship with a girl... Same thing could happen to the lady.
They don't want a normal life now but if their game actually works well like they say it does (performs a healing), they eventually will.

This was my thought. I'm more concerned that they both appear to be quite obese. Is this lifestyle unhealthy physically, leading to obesity? If it is, then it is clearly harmful to them both.

I think all fetishes (and many fantasies) can look like mental illness to anyone who doesn't share them. Why would anyone want to have pain inflicted on them? The first time any girlfriend of mine admitted to having otherwise relatively common rape fantasies, I really thought there was something wrong with her (why would any woman fantasize about being forced to have sex?).
All men I know have certain moments of infantilization. Arguably, even our fascination with breasts stems from that, but at the very least in many situations it's common for us to want caregiving maternal types around us. The man in the OP obviously takes this to an extreme that seems weird, but generally if the line between the fantasy and reality is not blurred and it's not rendering him and those around him unhappy, who am I to judge?
